Service Description

A transition or a stay-at-home plan is created with the senior (and their support system, as appropriate) that honors their preferences, abilities and needs. Individually tailored services may include bill organization/payment, hiring care providers, reorganizing the environment for ease of movement and safety, downsizing in preparation for moving, arranging estate sales and donations, finding transportation services for appointments, grocery shopping and engaging in the community, and coordinating doctor appointments. Services are $50.00 an hour.
